Finding housing can be difficult for individuals with special needs. Maybe they’ve been living at home, but are ready to move out on their own. Or perhaps they have reached adulthood, but still need some support at home. Fraser can help.
Fraser Community Living offers three different living options for adults with special needs: Supervised Living, Supportive Living and Independent Living. These distinct housing options are as unique as the people who live there. Our goal is to provide just enough support to ensure they can live comfortably on their own.
Fraser has 25 Supervised Living homes available to adults with special needs. These homes are located in neighborhoods throughout Hennepin and Anoka counties. The housing choice is for individuals who need more support at home and to participate in their community.
In Supervised Living, Fraser staff is on-site 24 hours a day. They help residents manage their daily needs and encourage them to work toward goals. Staff helps residents with grocery shopping, meal prep and attending events in the community.

Sweet Briar: Male only home
Sweet Briar is a rambler style home located on the border of Hopkins and St. Louis Park. The kitchen, dining room, living room, and all 4 bedrooms located on the main floor, with a laundry room, staff bedroom and office located in the basement. There are stairway entrances in the front and back of the home, with a patio set and swing in the backyard. The home is located in a mostly quiet and friendly residential area, conveniently located near the Knollwood Mall and many parks. Currently, 3 gentlemen ranging from 20 years old to 50+, live in the home. The individuals enjoy arts and crafts, watching TV, going out to eat, going outside when it is nice out, one of the current residents is even involved in adaptive sports! The house has 1-2 staff during awake hours when individuals are home, and 1 asleep overnight staff in the staffing pattern. There are also regular visits from the sites Registered Nurse.
Porcupine: Male only home
Porcupine is a split-level home located in a quiet cul de sac on the eastern side of Eden Prairie. Upstairs has 3 individual’s bedrooms, the main living area, kitchen and a bathroom with a tub. Downstairs contains one individual’s bedroom, bathroom, laundry area, and a small common area. The downstairs also contains the staff sleeping area and office for the Supervisor. There are 3 gentlemen currently residing at Porcupine ages 43-70, they enjoy outings such as going to the movies, shopping, out to eat, and going to see different sports teams play. On the weekends one to two individuals may go spend time with their families while others stay at the home. There is one individual that has a cat that lives at the home. The staffing is typically 1:4 with an asleep staff overnight. Porcupine house receives regular visits from our nursing staff.
Fraser Supportive Living offers person-centered services to adults living in their own apartments. Our team specializes in supporting people who have a developmental disability or intellectual disability diagnosis. Supportive Living services include a combination of 24-hour Emergency Assistance and Individualized Home Supports with Training. Services are based on the unique strengthens and needs of each person, with Fraser staff working to help them live as independently as possible. The services Fraser offers to support these individuals can be covered by either a DD or CADI waiver (or private pay at the same rate).

Woodstone:
Fraser has availability to provide ICS services at Woodstone to individuals living in their own 1 bedroom apartment home, or a 2 bedroom apartment with a roommate. Woodstone is in St. Paul and is located near recreation trails, several shops, and is less than 5 miles from the heart of St. Paul. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.

Alcott:
Fraser has availability to provide ICS services to individuals living in their own 1 bedroom apartment home, at Alcott. Alcott is in Minnetonka, located near Shady Oak RD and walking distance to several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment. County waiver funding is used to pay for staffing and services provided.

Fraser has availability to provide services to an individual living in their own 1 bedroom apartment home, or a 2 bedroom apartment with a roommate, at Parkside North. Parkside North is in Plymouth and is located near French Regional Park and several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.

Fraser has five Independent Living apartment buildings. These apartments are for adults, 18 years and up, with developmental disabilities who can live independently with little support. Independent Living buildings are located in Apple Valley, Bloomington, Hopkins, Minnetonka and Richfield.
Fraser staff mediate tenant issues, help with emergencies, plan activities and assist residents with engaging in the community. Residents pay 30% of their adjusted gross income in rent.
