Case Management Services Summary

Thomas Allen has been providing contracted case management since 1987. Since then, we have expanded our business, clientele, and expertise in a variety of areas. Thomas Allen has a reputation for providing top quality services, promoting fiscal responsibility in the development of the service plan, lead agency compliance, and achieving the highest standards of practice. We strive to provide person-centered case management by promoting the dignity and worth of each person we serve.

Thomas Allen serves people of all ages on DD, CADI, BI and Elderly Waivers. We hold contracts with Dakota, Hennepin, Ramsey, Chisago, Todd, Winona, Scott, St. Louis and Goodhue Counties. We also have Purchase of Service agreements with over a dozen other counties all over the State of Minnesota.

All of our case managers are licensed by the Minnesota Board of Social Work or meet the minimum qualifications of a social worker under the Minnesota Merit System. Thomas Allen provides thorough orientation for each case manager upon hire and our employees have expert-level knowledge of waiver policy. We also provide culturally aware practices and have language capabilities in Hmong, Spanish, German and Somali.

Managed Care Coordination Services Summary

Thomas Allen has been providing Managed Care Coordination since 2009. The role of a care coordinator is to assist members of Minnesota Health Care Programs with making informed choices about their health care services. The care coordinator is responsible for ongoing assessment, planning, referral, service coordination, monitoring and advocacy on behalf of the member. The care coordinator assists members in making progress towards health goals and coordinating care with the interdisciplinary team.

Thomas Allen contracts with Medica and Blue Cross/Blue Shield for care coordination for MSHO, MSC+ and SNBC products in Hennepin, Ramsey, Dakota, Washington and St. Louis Counties.

Our care coordinators are all Licensed Social Workers through the Minnesota Board of Social Work. They specialize in community resources, Medicare and Medical Assistance coverage as well as aging and geriatrics.
