Description of Services: Wraparound is a way to support families who face challenges related to mental health issues or other complex needs. It is a strength-based, family-driven, and team-based process. A child and family team develop an individual collaborative plan of care on the unique strengths of the child and family. Services are based in the child’s community and are reflective of the child and family’s culture. Wraparound teams meet to plan, brainstorm, implement, and monitor goals identified by the family and team members. Teams include both formal supports (paid service providers working with the family) and informal supports (friends or family members), all chosen by the family. Wraparound teams meet on an ongoing basis, as long as the family feels it is needed.

Process is based on the following ten core values:
1. Family Voice and Choice – Family is listened to and is a part of all decision-making
2. Team-based – Numerous participants providing needed support and ideas
3. Collaborative – Strength in people working together
4. Informal supports – Non-professionals bring unique strengths to the table
5. Community-based – Keeping the child at home and near loved ones is primary
6. Culturally competent – Family culture and traditions are respected
7. Strength-based – Planning emphasizes child and family strengths
8. Individualized – Solutions are based on child and family need rather than availability of services
9. Persistent – Team stays with the family as long as needed
10. Outcome-based – Planning focuses on achievable goals for child and family A group of agencies that plan and coordinate local services for children, youth and families
