We have 1 opening in our Family Residential Services Program, we hold a 245D license under Hennepin County in Champlin. We provide compassionate care to our residents with respect and dignity. We have over 10 years of experience working with people of disabilities and senior citizens. We provide services for people with mental health concerns, substance abuse, chronic medical conditions, and physical and developmental disabilities.
The services we provide include but are not limited to, bathing, grooming, meal preparation, medication management, setup or assistance, behavioral management, companionship, laundry, transportation, assistance with various appointments scheduling. We use person-centered planning to ensure the individuals we support live their best life. We do accept funding such as GRH, CADI, DD, MA, and private pay.
We support our clients in the family home environment. There is a community pool, playground, parks, walk trail and tennis in the neighborhood. One bedroom is available at lower level with a walkout, and the second bedroom is upper level. Each bed has its own private bathroom and shower. There are high-speed internet services. There is a shared living room at the main floor and lower level, and a large dining room.
