Healthpoint Housing with Services offers 24-hour Customized Living Group Home housing for adults 18 and older with disabilities: – mental illness, intellectual and developmental disability, and physical disability or chronic illness. We are committed to providing person-centered care that meets the needs of residents. Healthpoint Housing with Services provides care that makes our residents feel at home by optimizing independence as much as possible. Residents are actively involved in their care, direct their care, make their own choices, and have control over their lives as much as possible. Residents are encouraged to get involved and contribute to the community in a way that is meaningful to them.

We believe that no two individuals require the same formulaic services. We are committed to offering services that can be catered to each individual’s needs.

Services include one or a combination of the following, not limited to:

Assistance with ADLs (Bathing, Grooming, and Personal Hygiene)
Assistance with IADLs
Preparing Meals
Weekly restaurant takeaway or outing per residents’ preference.
Medication administration and management
Insulin administration, including sliding scale (insulin pens only)
CPAP & BIPAP management.
A call system for the person to summon assistance
Support with Housekeeping
Running Errands (Prescription Pick-Up, Dry Cleaning, Grocery Shopping, etc.)
Support with organization
Entertainment (Games, reading, coloring, movie night etc.)
Socialization
Arranging for or providing transportation
Train and promote independence as much as possible.

All rooms are private, with free Wi-Fi, HD cable TV, and a personal refrigerator. We have two immediate move-in at our locations, Brooklyn Center and Blaine.
