Pinnacle Services, Inc. provides Community Residential Services to support individuals of varying abilities to live full, purposeful lives. Residential Services are provided by trained professionals up to 24 hours per day, fostering a safe and supportive environment that balances independence with the support individuals need.

Powderhorn is a four-bedroom, co-ed, handicap accessible home in South Minneapolis, MN. The home is very active and individuals served enjoy being on the go.  The home is located next to a beautiful city park.

General Services

• 24-hour, awake supervision
• Menu includes 3 meals per day and unlimited snacks
• Laundry services
• Assistance maintaining a clean living environment and personal living space
• Private bedroom for each individual served
• Shared living room and kitchen
• Access to high-speed internet and cable television
• Preferences and choices of individuals served determine direction of their pursuits of meaningful life choices and desired outcomes
• Person-centered supports according to the unique interests of individuals served
• Services delivered in manner that promotes respect, dignity, and rights of each individual
• Support for goal attainment
• Assistance with personal cares and ADLs, as needed
• Opportunities and support in learning independent living skills

Medical Services

• Medication assistance and administration
• Coordination of medical service providers
• Transportation to medical appointments

Social and Community Engagement Services

• Regular opportunities for community outings of choice with transportation
• Encourage and support contact and visits with family and friends
