ADULT FOSTER CARE
24-hour supervision and protection for those with a primary diagnosis of mental illness experiencing difficulty in their daily lives. St. William’s has three residential adult foster care homes in Parkers Prairie.
Who is Eligible?
- Primary diagnosis of mental illness
- Experience difficulty in their daily lives
- Eligible for CADI waiver services
- Primary diagnosis of mental illness
- Experience difficulty in their daily lives
Services Available:
- 24-hour supervision/ protection, Single bedrooms
- Medication Monitoring / Assistance as needed to fit consumers’ needs
- Assistance with Independent living skills i.e. Personal cares, cooking, shopping, laundry and other household care maintenance tasks
- Assistance to and from appointments with transportation provided and updating appropriate doctors as needed
- Help setting up rehab goals and behavioral plans to meet consumers’ needs
- Assistance with budgeting
- Education about mental illness
- Community, social or Health services will be monitored, scheduled and recorded by the caregiver
Desired Outcome:
- Handle basic activities of daily living.
- Live in the least restrictive environment as possible.
- Participate in leisure activities.
- Set rehabilitation goals and plans.
- Manage mental health symptoms.
- Reduce the need for hospitalization.
A written service plan, placement agreement and vulnerable adult plan with measurable goals/objectives will be designed with the consumer case manager and guardian (if applicable) to meet the needs of each consumer.
