Housing Stabilization Services (HSS)
Housing Stabilization Services is a Medical Assistance waiver program designs to help people with disabilities and seniors find and keep housing. We support individuals overcome challenges and barriers that can make it hard to find housing, budget, interact with landlords and neighbors, and understand the rules of a lease.
Truwell helps:
Support an individual’s transition into housing
Increase long-term stability in housing in the community
Avoid future periods of homelessness or institutionalization.
1. TRANSITION – Plan for, find, & move into housing, may include:
Helping an individual think about preferred housing
Helping with the housing search & application process
Developing a budget
Understanding a lease
MN benefits application
Applying for cash and non-cash benefits to assist with gaps in income.
2. SUSTAIN – Keep housing after moving in, may include:
Education on tenant-landlord rights and responsibilities
Coaching to develop relationships with property managers and
neighbors
Training on how to be a good tenant
Lease compliance
Problem-solving to maintain housing stability
3. Eligibility
➔ Be 18 or older
➔ Have medical assistance (MA) coverage
➔ Have a documented disability or disabling condition
➔ Require assistance with at least 1 of the following areas (due to a disability or disabling condition):
Communication
Mobility
Decision making or,
Managing moods or behaviors
4. ➔ Be experiencing housing instability, such as:
Homelessness
At risk of homelessness
i. Including being doubled up, needing services to keep your housing, or you experienced homelessness in the past and now are being discharged from a correctional, medical, mental health, or substance use disorder treatment center without a permanent place to live
Moving out of (or moved out of in the last six months) an institution, nursing facility, or certain other group settings (Board and Lodge or Adult Foster Care, etc)
At risk of institutionalization
Meets an institutional level of care/eligible for the following waivers:
Brain Injury (BI)
Community Access for Disability Inclusion (CADI)
Community Alternative Care (CAC)
Developmental Disability (DD)
Elderly Waiver (EW)
